Passport applications may be obtained at the Southington Probate Court.

The fee for a new passport is $55.00 for an adult and $40.00 for a minor. The fee must be paid by check or money order. In addition, there is a $30 probate fee for processing the application, which may be paid by cash or check. Passports for children under 14 years of age must be accompanied by a long-form birth certificate and both parents must sign the application.

Passport renewal forms are also available. The fee for renewing a passport is $55.00 payable by check or money order. A passport may be renewed by mail under the following conditions:
1. Applicant can submit his or her most recent passport.
2. Applicant was 16 years or older when the most recent passport was issued.
3. Most recent passport was issued less than 15 years ago.
4. Applicant uses the same name or can submit proper documentation to reflect name change.
